About Us

Launch Housing has an ambitious and singular mission to end homelessness. We aim to get people housed, keep them housed, and be an influential voice on ending homelessness. We are a secular, independent provider of choice for clients and the go-to organisation on homelessness for government, media, philanthropy, private donors and the community.

The Role

In August 2018, as part of Victoria's Homelessness and Rough Sleeping Action Plan, the Department of Health and Human Services (DHHS) announced a range of new rough sleeper programs across metropolitan Melbourne as well as a number of regional centres across Victoria. As a result, the position of Intensive Case Manager has been created. The role is a part of the Towards Home Plus Team, a joint initiative led by Neami and partnered with Launch Housing, Wathaurong Aboriginal Co-operative, Uniting ReGen, Bolton Clarke and Melbourne City Mission.

The Towards Home Plus team offers a holistic, assertive outreach support service to people who are homeless and rough sleepers. They provide intensive, flexible and responsive client-focused support featuring a multidisciplinary team approach with the aim of assisting clients to successfully sustain housing, improve physical and mental health and develop connections within the local community. As a case manager within the team, you will provide case management and post-settlement support along with referral and linkage to relevant support services to your clients.

This role is employed and supervised by Launch Housing but will report directly to the NEAMI service manager for line management.

About You

You will be tertiary qualified in human services, social work or other relevant discipline and have a comprehensive understanding of the issues which those experiencing homelessness confront as well as the skills and aptitude in working for people who are marginalised and disenfranchised with complex needs.
