Launch Housing has an ambitious and singular mission to end homelessness. We aim to get people housed, keep them housed, and be an influential voice on ending homelessness. We are a secular, independent provider of choice for clients and the ‘go to' organisation on homelessness for government, media, philanthropy, private donors and the community.
We are currently seeking experienced and enthusiastic Case Managers (5 roles) to join our newly establishing Private Rental Assistance Program Plus (PRAP Plus) team at various locations across Melbourne's North and South / South Eastern suburbs.
The PRAP Plus program is an exciting new initiative funded by the Victorian Government to provide support to households accessing private rental properties that require additional practical support to establish or maintain their housing. PRAP Plus operates with a strong focus on sustaining tenancies in consideration of a vulnerable and disadvantaged client group and works to address needs and risk factors that make tenancies vulnerable.
The role of PRAP Plus is to ensure clients maintain safe, appropriate and sustainable accommodation by providing support to achieve identified goals and address issues that are impacting negatively on a tenancy. Case Managers work within an outreach based support model of service delivery and provides case management that includes assessment, case planning, case co-ordination and liaison.
As a Case Manager, you will provide case management and support to client with an aim to stabilize and sustain housing and prevent homelessness. You will work in an outreach capacity with clients assisting them to work through issues affecting or likely to affect their tenancy, assist them to connect to new services, supports, schools and their community more broadly.
To be successful in this role, you will have experience in a similar case management role or enthusiasm to deliver high quality and high impact services that compliment your tertiary qualification in social work, psychology, or a related discipline. Ideally you will also have an understanding of the housing sector and the Residential Tenancies Act. Knowledge and experience around the clients with complex needs will be highly regarded.
