About Launch Housing

Launch Housing has an ambitious and singular mission to end homelessness. We aim to get people housed, keep them housed, and be an influential voice on ending homelessness. We are a secular, independent provider of choice for clients and the 'go to' organisation on homelessness for government, media, philanthropy, private donors and the community.

Your impact on the community

We are currently seeking an enthusiastic person to join our case management team at our Cheltenham site in the role of case manager to support clients who are homeless or are at risk of homelessness. This includes singles, couples and families living in our transitional and crisis properties. The case manager works intensively with a small case load of clients, building engagement over time in order to meet case plan goals.

In addition you will:

  • Develop individual case plans in conjunction with clients and implement strategies that enable them to achieve their goals by building upon their strengths and focusing on solutions.
  • Assist clients to develop longer term housing options, both within and/or independently of Launch Housing's supported accommodation program.
  • Ensure clients maximise all government and community income assistance and support.
  • Initiate and maintain contact with vulnerable clients using flexible engagement strategies to achieve case plan goals.
  • Coordinate services required by the client and client's family and monitor their ongoing effectiveness.

About you

To be successful in this role, you will have a passion for working with people experiencing homelessness and a determination to deliver innovative and effective services of the highest possible standard to our clients.

You will also have the following:

  • A demonstrated ability to achieve quality client outcomes.
  • An understanding of the Victorian homelessness service system particularly with regards to crisis and social housing.
  • An understanding of case management practice and trauma informed principles.
  • Experience working with clients with multiple and complex needs due to dual diagnosis, domestic and family violence and/or psychological trauma.
  • Ability to engage and communicate effectively with clients, key stakeholders and external service providers.
  • An ability to empower clients using a strength based approach whilst setting boundaries.
  • Ability to respond to and manage critical incidents in a timely manner.
  • Experience working independently and as part of a dynamic, multi-disciplinary team.
