How you'll change lives

As a Case Manager, you will work with families and children for up to twelve months to deliver intensive support services based on a case-plan tailored to the needs of the family. The program assists families and children to positively engage in their communities of choice and, where appropriate, link families with resources in the community including education, training, health, employment and recreational opportunities. Your role will focus on the needs of children and actively and positively engage children with educational, health and support services. How impactful is that!

What you'll bring

To be successful in this role, you will have a passion for working with people experiencing homelessness and a determination to deliver innovative and effective services of the highest possible standard to our clients.

You will also have:

  • Experience providing case management and support in an outreach capacity with an emphasis on practical assistance to maintain housing
  • An understanding of the impact of homelessness on families
  • Ability to relate effectively and sensitively to families experiencing housing crisis
  • Ability to work both autonomously and collaboratively within a team environment
  • Valid driver's license
