The Role:

We are currently seeking an individual with case management or, related experience to join our Northern Outreach Team. Using an outreach case management approach, the Northern Outreach Team provides direct practical support to clients experiencing or at risk of homelessness; assisting them to access the specialist resources they need. A key focus of the role is assisting clients to access safe, affordable, appropriate and sustainable housing from which they can recover and participate in their chosen communities and networks.

About You:

If you have case management or related experience (including knowledge of assessment, case planning, case co-ordination and liaison), the ability to work alone in an outreach capacity and experience providing direct support, assistance and advocacy to clients presenting with complex needs, we want to hear from you! Professionals with qualifications in community services, social work, nursing, occupational therapy or other allied health disciplines are encouraged to apply.
