We are looking for a Team Leader to provide leadership to our FWC and HSP teams in Katherine.
The Lead Practitioner is responsible for the implementation, leadership and integration of the following programs: Financial Wellbeing and Capability (FWC) and Housing Support (HSP) in order for the programs to meet their objectives. Program portfolios may be subject to change within the contract period. This position has a particular focus on leading and mentoring program staff, applying professional knowledge and support whilst providing direct services and managing Financial Counselling client work.
CCNT provide integrated Financial Counselling, Capability and Resilience hubs throughout the NT. The Katherine team includes a Lead Practitioner Financial Wellbeing, Capability and Housing Support and Financial Wellbeing and Capability Case Managers, who contribute to improved outcomes for vulnerable people, families and communities by:
CCNT implements a case management approach to break the cycle of homelessness through empowering clients to effectively problem solve and manage their accommodation. CCNT uses an early intervention approach that supports individual clients to regain control of their circumstances including; housing, financial, social and community issues that are placing them at risk of homelessness.
Through assertive case management Housing Support Case Managers will provide clients with support to address the multifaceted challenges they face in maintaining their home. This support may be through assessment of life skills, role modelling, referral to specialist agencies as required, coordination of supports, and advocacy with internal and external agencies. CCNT's case management framework adheres to the Standards of Practice established by the Case Management Society of Australia and New Zealand and Affiliates.
