About the facility

Barwon Health's Surfcoast Continuing Care Service provides community based treatments to those aged between 25 - 65 years of age, experiencing serious and complex mental illness.

The team is multidisciplinary and covers a catchment area starting from Waurn Ponds all the way to Aireys Inlet.

Barwon Health MHDAS also provides innovative clinical programs and integrated models of practice that are client centred, accessible and family inclusive.

Service delivery includes the following:

• Comprehensive crisis assessment
• Crisis management and treatment
• Case management of complex issues
• Continuing care
• Mobile support and treatment

About the role

Barwon Health is excited to present an opportunity in Continuing Care Service delivery, working within the Surfcoast Community Mental Health Team.

We are searching for people with experience in public mental health service delivery such as:

• Assessment
• Treatment Planning
• Treatment delivery
• Use of evidenced based tools to determine effectiveness of treatment
• Family inclusive practice
• Family Violence practice within the MARAM framework
