About the facility

Barwon health's Mental health, Drugs and Alcohol Services (MHDAS) provides integrated and recovery orientated mental health and drug and alcohol assessment, treatment, harm reduction and community and family support to people of all ages within the Barwon Region currently experiencing, or at risk of experiencing mental health and/or drug and alcohol issues. Our service is internationally recognised for its innovative clinical programs and integrated models of practice that are consumer centred, accessible and family inclusive. We are fortunate to enjoy excellent internal relationships and have developed excellent partnerships with local community that provide for seamless integrated care for our clients.

About the role

To provide recovery orientated service delivery to people utilizing mental health, drug and alcohol services (MHDAS)

Major activities of the Clinicians Case Management role includes but is not limited to:

  • Working within a multidisciplinary team that provides a range of psychiatric case management services to Aged Psychiatry clients, their carers and families within a community setting. This may be a nursing home, residential service, their own home and/or the carer’s home
  • Providing client’s meeting the target group; comprehensive assessments, crisis intervention, outreach case management, formulate and review Treatment and Recovery Plans in collaboration with the client and other parties concerned with the client’s care and refer to other service providers when appropriate
  • Undertaking a clinical caseload which is decided upon in consultation with the Consultant Psychiatrists and/or the Clinical Co-Ordinator and/or the Senior Clinician
  • Ensuring the client has an acute presentation plan entered on TCM data base this is realistic, achievable and includes options for action outside normal business hours and weekends
