About the facility

Barwon Regional Aged Care Assessment Service (BRACAS) provides comprehensive assessments to older persons in the Barwon Region to assist them to identify their care needs and gain access to appropriate care. These include approvals for Commonwealth Government subsidised Care Services. BRACAS is a multidisciplinary service located at McKellar Centre in North Geelong.

About the role

Are you looking for a rewarding career that will make a difference to the ageing population? As an Assessment Clinician your role will be to conduct a Comprehensive Assessment to assist the consumer with choice of appropriate service and information to meet their needs and preferences. This may include facilitating access to Residential Care, Residential Respite, Home Care Package or broader community services for example: Regional Assessment Service, Mental Health Services, NDIA and CHSP.

Duties of your role include, but not limited to:

  • Informing client's primary carers and liaise with referrals source, to provide professional advice and recommendations
  • Utilising problem solving techniques that are both realistic and achievable to obtain the clients optimum level of functioning
  • Assisting vulnerable clients approved for Home Care Package who require assistance to access a package
  • Continued work in a fast changing environment whilst providing support and professional advice to clients/carers/stakeholders.
