About the role:

Anglicare has an excellent career opportunity for a full-time Case Manager or Mental Health Practitioner in our Emotional Wellbeing for Older Persons program in the Central and East Sydney region. This program aims to provide psychological wellbeing support services for residents in Residential Aged Care Facilities (RACFs) across the Central and Eastern Sydney region. You will be working across Residential Aged Care homes and would suit the person who like to work autonomously, as well as having the support of a large team

In this role you will be providing assessment and psychological interventions to meet the needs of Anglicare Emotional Wellbeing for Older Persons (EWOP) program participants with mild and moderate mental health needs.

This role is remunerated under the SOCIAL, COMMUNITY, HOME CARE and DISABILITY SERVICES industry award (SCHADS).

Key responsibilities (not limited to):

• Working with participants to meet mental health needs.
• Facilitation and delivery of face to face and telehealth interventions to individuals, as well as groups, to support best outcomes for participants
• Use of a variety of tools such as counselling, advocacy and coaching to assist in re-engaging the participant with their family and the community.
• Working collaboratively with service providers, carers and other organisations.
• Capacity building with aged care staff across the region
• Engagement with staff of RACFs to market and promote the EWOP program

About you:

• Relevant tertiary qualifications for the professions of Psychologist, Social Worker, Registered Nurse, counselling, diversional or occupational therapy
• Current membership to a professional body (e.g. AHPRA, AASW, PACFA or equivalent) is desirable
• Experience in working with people with mental health challenges and/or experience in the aged care sector
• Current NSW Driver’s Licence and own comprehensively insured vehicle
• Excellent written and verbal communication skills
• Valid NSW Working with Children’s Check for paid employment purposes
