Better Connected Community Services is a not for profit organisation serving both St George and the Sutherland Shire.
The position includes to support older people, their families and carers by providing an entry point to the aged care packages by assisting them in accessing information and appropriate services. This is a case management role in the operation of the My Aged Care Portal, updating of consumer records, care planning with clients, assessments and data reporting.
Current contract is till June 2022 with high possibility of renewal
Essential experience or skills
- Related certificate qualifications or minimum of 2 years’ experience in Aged Care services
- Demonstrated knowledge of the New Aged Care Quality Standards
- Demonstrated knowledge of Home care Packages and Commonwealth Home Support services
- Experience in customer service with people from CALD backgrounds and people with complex needs
- Experience in managing My Aged Care Portal
- Experience in using a client management system
- Demonstrated computer skills with capacity to learn and use electronic reporting systems
- Experience in client assessments and reviews
- Good organisational and problem solving skills
- Strong Communication and interpersonal skills
- Ability to manage competing priorities
- Proven ability to work in busy environment, skills in time management and ability to prioritise
- Ability to work effectively in a team
- An understanding of Work Health Safety issues
- Current NSW drivers’ license
