We are the largest, most comprehensive Aboriginal Medical Health Service in Queensland, and Australia’s second oldest. Role Functions:
Effectively engage with residents (and their family, if applicable) requiring secure housing and/ or improvements in their mental, physical or social health. The HWW provides tailored case management services to secure and maintain stable long term housing, addressing any health needs, and linking residents into services in the community. The HWW is responsible for developing, implementing and executing the personal care plan of residents. The HWW ultimately ensures residents transition into sustainable accommodation with security of tenure, and are equipped with the skills to maintain their tenancy.
Organisational Contributions:
Diligently and accurately record ongoing client information, case notes and goal tracking as part of high quality case management practices. The HWW will act as a navigator of the health and social services system to improve access and quality of service responses to the residents. The role will lead internal and external case planning activities with other agencies and stakeholders.
In addition the HWW will work to coordinate and deliver therapeutic brief interventions to encourage residents in achieving their goals and provide crisis response care as required. The role will focus on advocating and empower residents to secure stable long term accommodation by assisting in removing barriers. The HWW will provide direct support alongside the residents in resolving social welfare matters (i.e Centrelink, housing etc.). The aim will be to act as a advocate, ensuring residents are receiving best quality services and are actively involved in all decisions that affect their lives and treated with respect, dignity and empathy.
