About the Role:

The Care Coordinator will effectively engage with Aboriginal and Torres Strait Islander families that require family support services. The role will conduct initial comprehensive assessments, utilising narrative therapy and family-led approaches to identifying the real and genuine needs of a family.

The family-led assessment approach will spearhead into the development of a ‘Single Family Care Plan’, with corresponding goals. This role will act as the main ‘point-of-contact’ for the family and work in partnership with the family to help facilitate supports in achieving their goals. The role will lead internal and external case planning activities with other agencies.

The role will operate heavily through a community outreach/in-home engagement approach to remove any access barriers to families. The aim will be to act as a family advocate, ensuring families are receiving best quality services and are actively involved in all decisions that affect their lives and treated with respect, dignity and empathy.

About You:

Essential:

  • Certificate IV qualification or higher in Social Work/Human Services/Behavioural Science /Psychology/Criminology/Allied Health/Occupational Therapy/Sociology/Public Health or relevant field.
  • Demonstrated ability to apply a high level of practice judgement when working with clients in setting and achieving goals.
  • Ability to work with a culturally and socially diverse group of clients, and to apply a culturally responsive framework to guide interventions, with the support of staff and management.
  • Ability to work with diverse people and presentations in providing case managed social work driven supports.
  • Demonstrated ability to record accurate and consistent case notes.
  • Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services.
  • Demonstrated experience in overseeing and coordinating case management processes and procedures. 
  • A willingness to travel across the ATSICHS Brisbane and Logan region of services and work across different sites.
  • A willingness to provide social work supports and engagement to patients outside of a clinic environment. 
  • Working with Children’s Card (Blue Card) or be eligible to apply.

Desirable, but not mandatory:

  • Bachelor qualification in either Social Work/Human Services/Behavioural Science/Psychology/Criminology/Allied Health/Occupational Therapy/Sociology/Public Health or relevant field.
  • Previous experience in a family support and or advocacy role.
