About The Role

Effectively identify the needs of young people in delivering targeted and goal driven supports that focusses on their genuine needs and aspirations. The Case Manager will directly assist young people to access a coordinated wrap-around suite of services with the aim to achieve outcomes of the four (4) program objectives.

Diligently and accurately record ongoing client information, case notes and goal tracking as part of high quality case management practices.

About You

Essential:

  • Certificate IV qualification or higher in Social Work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention, Management, Project Management/Coordination or relevant field. 
  • Previous experience in case management and coordination activities using a client information system.
  • Demonstrated ability in working with multiple agencies in providing access to holistic care for clients.
  • Demonstrated ability to meet targets and performance outcomes.
  • Knowledge of the supports available to young people in the areas of mental health, education, employment and housing.
  • Ability to communicate sensitively and effectively to young people through the use of decisive listening, influencing and persuasive skills.
  • Current drivers licence.
  • Willingness to travel across the Brisbane region.
  • Willingness to complete 1 to 2 night outreach shifts a week.
  • Working with Children’s Card (Blue Card) or be eligible to apply.

Desirable, but not mandatory:

  • Previous experience in social or health service.
