About the role

As a key member of the ATSICHS Brisbane team, the Care Coordinator is expected to personally contribute to the shaping and achievement of ATSICHS vision and goals. The Care Coordinator will provide three (3) key functions:

Provision of quality Care Coordination services including improved access to CCSS program:

The Care Coordination and Supplementary Services Programme (CCSS), in the Indigenous Australians’ Health Programme, supports activities to improve the prevention, detection, and management of chronic disease in Aboriginal and Torres Strait Islander people. Better management of these chronic conditions is a key factor in closing the gap in life expectancy between Indigenous and non-Indigenous Australians within a generation.

Implementation of the CCSS program whilst operating within quality assurance framework:

The Programme improves access to primary health care, integration of primary care with allied health and specialist care and enables other ancillary and follow-up activities necessary for effective chronic disease management. The program aims to overcome barriers such as lack of transport, difficulties with medicines compliance and promotes continuity of care.

Actively contribute in developing a flourishing team:

Support fellow staff in a team environment to build collegiality and a sense of belongingness within the team and ATSICHS family.
