About the role

ARRCS Community Care programs deliver disability and aged care services 365 days across the Northern Territory. We now have a permanent opportunity in Alice Springs for Support Coordinators on CHSP in-home care programs.

The Support Coordinators are responsible for coordination and case management of clients and participants within the funding streams of CHSP, NDIS - CA, DVA - VHC.

Skills & experience

  • Certificate IV in Aged Care / Individual Care / or Disability Care
  • Knowledge and experience of the Support Coordination function and why it is important in building a Clients social capacity and to the overall success of the funded programmes.
  • Knowledge of relevant service providers across mainstream and other community service sectors, including mental health, dementia services and disability providers.
  • Demonstrated experience in working as a team within human services – Aged Care, Disability, Mental Health, Housing or experience in another relevant sector
  • Demonstrated ability in the use of MS Word, Excel and other online client reporting systems
  • Demonstrated ability to respond and adapt to competing priorities whilst working unsupervised
  • Demonstrated ability to empathise with Aged People, their families and Personal Carers
  • National Police Check and/or ability to obtain and hold
  • Bachelor in Health, Social Work, Psychology, Occupational Therapy or Social related Discipline (Desirable)
