ABOUT US

Arafmi Ltd is a community organisation providing support for families, carers and people with a mental health condition for the past 40 years.

We are looking for people that have experience in supporting individuals with complex mental health issues and/or challenging behaviours. To be successful in this role, you must have relevant working experience and understanding of NDIS practices and individuals living with mental health issues.

The Arafmi Ltd Specialist Support Coordination role is designed to assist participants to access supports from their NDIS plan that will assist them to build the skills to increase their capacity to maintain relationships, live more independently and be included in the community. Arafmi Ltd supports each participant’s right to practice and express their culture, values, beliefs and sexuality while accessing supports.

KEY RESPONSIBILITIES

Service Delivery

Specialist Support Co-ordination is delivered utilising a specialist approach, necessitated by high and complex needs or high level risks in a person’s situation.  Specialist Support Co-ordinators address highly complex barriers impacting on the ability to implement a participant’s plan. Key responsibilities include:

  • Implement client service delivery ensuring NDIS Support Coordination objectives are met
  • Implement NDIS service provision processes and provide advice and recommendations for organisational or service delivery change
  • In collaboration with Line Management, identify possible areas for growth and opportunity within the NDIS framework
  • To assure that no conflict of interest will arise, specialist support coordination is a distinct position, sitting separately from other Arafmi service supports.

Team Work

  • Establish and maintain professional and effective working relationships with individuals living with psychosocial disabilities, professional colleagues and external service partners.
  • Maintain a safe work environment by following WH & S policies and procedures.
  • Uphold and follow the Arafmi Ltd Code of Conduct, maintain a high level of privacy and confidentiality, and work within the policies and procedures of the organisation.
  • Actively participate in all staff meetings, training and Arafmi Ltd development activities.
  • Record information into client files as per organisational procedures.
  • Complete other duties as requested.

As stated we are looking for applicants with experience working with individuals with complex mental health issues and require all applicants to respond to the following selection criteria to be considered for the role:

KEY COMPETENCIES

  • High level knowledge and experience in the mental health and disability support sectors at a state and national level.
  • Demonstrated knowledge and understanding of the NDIS and its implementation to date.
  • Demonstrated experience in managing complex projects within community-based support services.
  • High level communication and report writing skills and commitment to working collaboratively with stakeholders.
  • Ability to independently navigate and sustain necessary supports required for client crisis management

Successful applicants must also possess:

  • Relevant tertiary qualification, ie Bachelor of Social Work, Psychology, Occupational Therapy or other allied health (such as Developmental Educator, Social or Health Science degree) and able to obtain either AASW or AHPRA registration
  • Minimum of 5 years’ experience in the community sector and project management will be highly regarded
  • Access to a personal registered motor vehicle, with full comprehensive insurance
  • Current drivers licence
  • Demonstrated experience and skills in working with individuals with psychosocial disability and their carers
  • Sound computer and administrative skills, including file noting and record keeping

Successful applicants will also require the following:

  • It is mandatory for all employees to undergo criminal history screening in order to hold a Yellow/Blue Card
  • Possess an NDIS Worker Orientation Module Certificate
  • All staff employed by Arafmi Ltd are required to observe its policies and procedures.
