About the Role?

The role exists to facilitate quality support arrangements for the people we support, that are scheduled in the home and in the community. The Support Coordinator is responsible for ensuring supports are provided in accordance with a person’s individual plan, through the appropriate matching of Support workers, rostering of scheduled services and the ongoing review of rostered supports.

As an annecto team member you will show a commitment to supporting our clients to reach their goals in life and provide support services in a manner that respects their quality of life, dignity and human rights. The incumbent will also ensure reporting, documentation and records requirements are completed as per annecto policies/procedures and Government legislation.

Requirements:

  • Demonstrated commitment to the Purpose and Principles of annecto - the people network;
  • Knowledge and understanding of the National Disability Insurance Scheme, the disability and Aged care sector;
  • Understanding and commitment to person centered approaches;
  • Demonstrated ability to work efficiently with culturally diverse groups;
  • Demonstrated ability to work autonomously with a high degree of initiative;
  • Excellent communication skills, both written and verbal;
  • Previous experience in support coordination in a community setting;
  • Well-developed time management skills, and the ability to prioritise;
  • Current unrestricted Victorian Drivers licence.

What would be great to have?

  • Experience working in a similar capacity in a community based not-for-profit organisation.
  • Knowledge of client data bases e.g. Carelink
  • Training and experience in Person Centered Planning
  • Knowledge of DEX reporting
  • Ability to speak community languages
