Due to growth within the Home care packages team, we are seeking to recruit an experienced Case Manager. Reporting to the Co-ordinator- client services and working within the Home care packages team, the incumbent will assist and support eligible recipients who have complex care needs and provide solutions and alternatives to residential care.
The incumbent will also promote engagement with all key stakeholders, and ensure homemade packages are planned, implemented and monitored in consultation with individuals, their carers, service providers and other significant people in the person’s life.
