About the Position

You will work as a collaborative member of the Aged Care Team. As the Care Coordinator you will also identify tailored care solutions to enable our consumers to achieve their goals and enhance their quality of life; oversee individual consumer's Home Care Package Budgets; and connect professionally with family members and external organisations to serve the consumer's best interests. You will be regularly travelling to meet consumers in the community/homes and liaise with the extended internal and external contacts to ensure the smooth delivery of care services. You will also strive to make a positive difference to families and achieve consumer satisfaction.

You will uphold the values of Anglicare NT and provide quality services within the scope of the position and associated delegations.

Please note: It is a requirement that prior to commencing in this role, you must possess a Working with Children Clearance (Ochre Card), a National Criminal History Check (within 3 months) and a current Northern Territory driver's licence.

Experience of working closely with and having a strong understanding of Aboriginal & Torres Strait Islander culture would be considered an advantage.
