As a collaborative member of our Aged Care team, you will identify tailored care solutions that enable our consumers to achieve their goals and enhance their quality of life. You will work within CHSP budgets and engage professionally with families and external stakeholders to meet the consumers best interests. You will regularly provide outreach support meeting with consumers both in the community and in their homes.
As the Care Coordinator you will hold a Diploma in Community Services or Case Management and have extensive experience (minimum 3 years) experience in the Aged Care Sector. You will have relevant experience in care coordination, care management and assessment and a demonstrated ability to provide delivery of services to the aged within their homes. You will also have experience supervising small teams with a high level of motivation and a team orientated approach.

Please note: It is a requirement that prior to commencing in this role, you must possess a Working with Children (Ochre Card), a National Criminal History Check (within 3 months) and a current Northern Territory driver’s licence.
