At Anglicare Victoria our focus is on transforming the futures of children and young people, families and adults. Our work is based on three guiding pillars, Prevent, Protect, and Empower. We offer a comprehensive network of services and seek to ensure the provision of high quality services that will bring about significant improvements in the life experience of the young people, children and families/caregivers with whom we work.
Anglicare Victoria (AV) offers flexible and tailored support services as part of the Family Preservation & Reconciliation (FPR) Response, a Department of Families, Fairness & Housing (DFFH) state-wide initiative. The services within the FPR use various approaches including early intervention and assistance strategies, targeted and specialist support and continuing care pathways. AV provides FPR through short-term intensive support services and long-term transitional support services, based on the needs of the children and their families.
The new Future Pathways team compliments the suite of family supports within the FPR model. This program works collaboratively with children, young people and families to provide an immediate and tailored intervention with the Family Preservation and Reunification (FPR) response. The Future Pathways approach is innovative and built on practice elements such as motivational interviewing, communication skill building and goal setting to respond to identified need and achieve real and demonstrable outcomes for vulnerable families. In working collaboratively with children, young people, parents and carers they seek to address key intersectionality issues with families, children and young people (e.g. substance use, mental illness, low income, social isolation) as identified by each regional FPR Response.
The role of the Future Pathways Practitioner is to:
