This is an exciting opportunity for an experienced practitioner who enjoys working within a 'whole of family' approach. Future Pathways is an innovative approach, built on the Practice Elements, working collaboratively with children, young people and families to assist families in achieving their priorities and key goals.
The Practitioner - Future Pathways position sits within the FPR (Family Preservation & Reunification) Response program in the Western Metro region and is complimented by two other Evidence Based Models: FFT-CW (Functional Family Therapy Child Welfare) and Rapid Response. The FPR Response delivers intensive support to families that DFFH Child Protection have assessed as requiring intensive and tailored support.
The Practitioner - Future Pathways supports families where there is a child or sibling group with a FRO (Family Reunification Order) and a reunification plan in place; where there have been reports raising concerns about an unborn child or when families have completed a four week urgent response with Rapid Response, in order to provide longer term family preservation work. You will provide support to families utilising a suite of Practice Elements for which training is provided by DFFH and Anglicare Victoria. Upon exiting the program the aim is to ensure that families have what they need to sustain positive change.
Please Note: It is mandatory for the successful applicant to hold a current Working with Children Check and be prepared to undergo a National Criminal History Check prior to commencement. AV employees are required to either be fully vaccinated against Covid 19 or have a medical exemption. AV will require evidence that you are compliant with these requirements.
