We are seeking a qualified part-time (0.8EFT) Targeted Care Package Case Manager to join our friendly and supportive Targeted Care Package (TCP) team at our Dandenong office. We are looking for someone with case management experience with exceptional teamwork and communication skills who is able to effectively lead care teams of multi-disciplinary professionals to deliver goals outlined in TCP’s.
The part time Case Manager role will work alongside Youth Workers and other professionals in the delivery of TCP’s. The position objectives are:
