About the program

The Survive and Thrive program is a community-based individual and family support service that aims to build resilience in families who may be experiencing significant vulnerability. The program provides early intervention and prevention services to improve children's development and wellbeing.

About the Role

The Family Case Manager aims to build family capacity by providing support to families using partnership, strengths-based and family-centred approaches. Support is provided in both group settings and on an individual basis.

You'll love this opportunity to draw upon your knowledge base and training in equal measure, as you:

• Work in culturally competent ways with Aboriginal families and within Aboriginal communities
• Develop relationships and network with Aboriginal service providers
• Deliver an integrated support service for children 0-12 years and their families, with a focus on prevention services to improve experiences of social and economic exclusion.
  Develop relevant case plans to support the family to achieve their goals
• Assist with community driven activities and projects that promote community participation enhance parenting experience and meet program objectives

You'll be drawing upon your experience and knowledge of the child and family sector on a daily basis in this role, as well as your experience in dealing with the Aboriginal community, so those top-level communication, negotiation and diplomacy skills will be needed. You will also need a diploma qualification in Community Service or a related discipline.
