THE TEAM AND THE ROLE

This position forms part of our Support Work team that specialise in supporting people with psychosocial disability. The Support Worker position focuses on assisting participants to meet the goals within their individualised NDIS plan. The NDIS aims to provide people with choice and control over how, when and where those supports are provided. Providing outreach support to NDIS participants will be your main duty, however additional duties, should you be interested, may involve attending Alacrity Health day trips, retreats, and other fun activities.

Our community support team (CSW) provides 1:1 or group assistance, utilising their experience to give trauma-informed care to our participants with mental ill health. This is done flexibly and in consultation with family/friends and the wider care team.

Our peer work team (PSW) provides support work to the community, utilising their lived experience above all else to provide trauma-informed care. As a PSW, you provide mental health support like a community support worker, however with your lived experience at the core of your expertise.

Please note that for the outreach component of your role, you will need to use your own vehicle. Travel reimbursements for this form part of the remuneration.

KEY RESPONSIBILITIES

Service delivery and development

  • In collaboration with your supervisor, coordinate the initial needs assessment and intake of all consumers into the service
  • In conjunction with the Team Leader, participate in partnership development, creating pathways for Alacrity Health consumers into community life
  • Provide peer support, alongside direct practical support as needed, to consumers so that they gain/maintain independent living skills. This includes assistance by sharing skills in cooking, nutrition, personal care, budgeting skills, shopping, maintaining the home, utilising public transport
  • Working collaboratively with consumers to identify their needs, set goals and develop a plan to meet those goals
  • Having fun

Administration

  • Record case notes as required
  • Completing reports and client assessments

Professional development

  • Participate in Practice Development sessions (supervision) with manager
  • Actively work to improve identified skill areas

In addition, you will need:

  • A tertiary or other qualification, or experience to a similar level in the areas of health; welfare; allied health or housing
  • An NDIS Worker screening check will be undertaken prior to commencement
  • Police check
  • Current Working with Children Check
  • Australian Working Rights
  • Current Australian Drivers license and vehicle with insurance
  • Computer literacy
