About the roles

These are currently fixed term opportunities to 30 June 2025, with possibility of extension.

  • 4 x Full time positions Perth Metro North
  • 4 x Full time positions Perth Metro South
  • 1.5 positions Wheatbelt (2 x part time roles OR 1 x full time and 1 x 0.5 FTE; we are happy to consider applicants Perth based with regular travel to Wheatbelt OR Wheatbelt residents)

We have been advocating for older Western Australians across Western Australia for over 26 years now, and currently provide Aged Care Advocacy support in the metropolitan and several regional areas.

The primary responsibility of the Supportive Outreach Coordinator (SOC) is to provide specialist and intensive assistance to help people within the Care Finder target population to understand and access aged care and connect with other relevant supports in the community. The role will include approved travel within your designated service delivery area and also involves phone and direct (face to face) client contact to deliver information and support coordination. SOC’s will be expected to deliver information sessions on the Care Finder Program to community groups, specifically targeting vulnerable older Western Australians.

The positions report directly to the Team Leader, Supportive Outreach and works collaboratively with the Programs Team and wider Advocare team to further Advocare’s objectives

What to expect in this role

  • Perform assertive outreach client service activities providing information and support coordination to eligible clients
  • Supporting clients to interact with My Aged Care
  • Supporting clients to identify and engage with required support services
  • Provide and create referral pathways
  • Facilitate presentations / information sessions with older persons and the wider community regarding the Care Finder Program
  • Enter all data / case notes correctly into the client data base system in an accurate and timely manner
  • Maintains appropriate records and documentation consistent with the relevant policies and instructions of Advocare
  • Facilitate client feedback where appropriate
  • Build respectful, working relationships with other service providers, local councils and relevant stakeholders in the sector
  • Pursue opportunities to improve service and deliverables in your areas
  • Undertakes training / professional development and shares learning

Work Related Requirements

Essential:

  • Advanced knowledge of My Aged Care systems and processes
  • Demonstrated understanding of the aged and community care sector
  • Sound knowledge of holistic client service support networks and organisations
  • Strong stakeholder management skills with proven networking relationships in client support services, including but not limited to mental health, legal aid, drug and alcohol services, housing and homelessness support, community groups
  • Excellent written and verbal skills
  • Well-developed facilitation/ negotiation skills in working with clients and engaging support services
  • Confident facilitation/ presentation/ public speaking skills to deliver group education sessions
  • Commitment to delivering quality customer service, working empathetically with clients to ensure we deliver rights-based and client-focused services
  • The ability to work well within a small team with minimal supervision
  • Strong computer literacy, outlook, word and power point and client data management system
