Adelaide PHN is seeking a motivated health professional with experience in the Primary Mental Health sector to join our team as our Head to Health Project Manager.
Adelaide Primary Health Network (Adelaide PHN) is an independent, not for profit, primary health care organisation based in Adelaide.
We aim to improve the health and wellbeing of the Adelaide community by coordinating and integrating health and community services and by working in partnership to identify local needs and create strategies in response to keeping people well and out of hospital; improving their experience AND their health outcomes – to deliver on our vision of A Healthier Adelaide by 2030.
The new Head to Health Project Manager is a key leadership role within our Mental Health and AOD Team and will support the establishment, ongoing development, and implementation of the Head to Health model as a platform for mental health system navigation, including phone line, Hub/s and website that are integrated with PHN commissioned and other primary care services in South Australia.
The Head to Health Project Manager will:
Reporting to the Executive Manager Mental Health and AOD, initially the role will work closely with the CRU Operations Manager to implement the Head to Health National Phone Line.
To be successful in this role you’ll need:
You may have had experience in previous roles in the primary health sector such as: Mental Health Lead, Project Manager, Service Manager, Implementation Manager, Senior Project Manager, Project Lead.
