About the job 

In Albany we offer integrate d services to customers across accommodation, community and employment.  The primary purpose of this role is to support customers to develop and maintain skills to achieve independence. This includes implementing and monitoring person centred plans that work towards outcomes in alignment with National Disability Standards.

We have 4 casual positions available with the potential of part time/full time permanent employment.

Some key duties:

• Deliver a wide range of domestic supports including personal hygiene, personal care, health and well-being

• Developing activities of interest for the customer you support
• Assisting customers to explore opportunities in volunteering and employment
• Work with a person-centred plan
• Mentor customers to learn and develop their independent living skills such as budgeting, grocery shopping, cooking skills, help around the home etc.

What we are looking for:

• Availability to work during the week and some weekends
• Demonstrated passion to support the community and alignment with our values is required.
• A keen interest in outdoor activities including walking and cycling
• Demonstrated ability to build effective relationships with customers
• Valid full WA drivers licence and ability to drive our fleet vehicle
• Willingness to use your private vehicle (aligned with Activ’s motor vehicle allowance)
