Release Support Worker - Bunbury

Accordwest is a vibrant, not for profit, community based organisation. We are offering a unique and fulfilling employment opportunity for a suitably qualified, innovative and motivated person for the position of Release Support Worker.

As a Release Support Worker, you will be responsible for the delivery of proactive, person centred reintegration support services to clients in Bunbury Regional Prison and the local community. Our Release Support Workers case manage clients for up to six months pre-release and 12 months post-release. Additionally, we deliver single services to clients on remand, life skills group programs and transitional accommodation supports. Our goal is help improve the lives of our clients and contribute to a reduction in recidivism in our community.

In this role, you will:

  • Provide reintegration support to clients within a trauma informed, case management framework
  • Assess client needs and risks and develop goal orientated Support Plans
  • Empower clients to move through the stages of change, address barriers for their reintegration and engage in pro-social activities
  • Facilitate group programs and deliver one off services
  • Apply trauma informed principles in all aspects of your work with clients
  • Develop and maintain productive working relationships with external and internal stakeholders, including employers, education providers, not for profit and community agencies
  • Participate as an active team player within a multi-disciplinary team, providing support across the team when required.

Essential Requirements

  • Demonstrated experience providing person centered case management services to vulnerable clients
  • Demonstrated experience in assessing client needs and risks and developing case planning to address barriers and support clients to achieve their goals
  • Demonstrated experience in empowering clients to move through the stages of changes, build life skills, overcome challenges and increase engagement in prosocial activities
  • Demonstrated experience in monitoring service outcomes, undertaking research, preparing case management and service reports for funding bodies
  • Demonstrated experience in improving outcomes for clients with complex needs
  • Demonstrated ability to work collaboratively in a multi-disciplinary team to achieve outcomes
  • Demonstrated experience in building and maintaining productive relationships with internal and external stakeholders
  • Demonstrated experience in delivering quality support services and maintaining professional boundaries
  • Demonstrated ability to work autonomously, comply with organisational policies and procedures and a strong understanding of maintaining personal and client safety at all times
  • Excellent interpersonal skills
  • Highly developed written and verbal communication skills
  • Excellent IT skills with a demonstrated ability to operate client management systems and reporting software
  • Self-motivated team player who enjoys working within a challenging, fast-paced community services environment
  • An ability to obtain and maintain a Working with Children Check, National Police Clearance, Drivers Licence, First Aid Certificate and Department of Justice Security Clearance.

Desirable

  • Tertiary qualification in Social Work, Psychology or another relevant field of study
  • Understanding of reintegration and rehabilitation services in Western Australia
