The intent of the Out of Home Care program is to provide a secure, safe and supportive home environment for children and young people who cannot reside with their families. The program includes Family Group Homes, Specialised Foster Care and Transitional High Needs.

Our Case Managers are based in both the Bunbury and Mandurah offices and service the South West and South Metro/Peel areas. 

In this role:

  • You will provide case management to children in care, working closely with the Department of Communities (CPFS).
  • You will assist with overseeing the quality of care provided in the home(s) including support to Therapeutic Carers and Foster Carers, in accordance with AccordWest and the Department of Communities (CPFS) policies and procedures.
  • You will ensure all reporting requirements are met in line with AccordWest and Departmental requirements.
  • You will ensure that a joint case management approach is maintained between AccordWest and the Department, along with other agencies and relevant professionals for assigned cases.
  • You will maintain all documentation in relation to the goals set out in each child’s individual care plan.
  • You will engage with children and young people who present a range of complex behaviours, who often have difficulties forming lasting relationships.
  • You will be supported in your role through comprehensive training and development, and Clinical Supervision, specific to our Out of Home Care program.

Essential requirements

  • Tertiary degree in Social Work, Psychology or other relevant field of study.
  • Sound clinical and case management skills, with experience in the OOHC field preferred.
  • Capacity to participate in the ongoing development, implementation and evaluation of programs across the service region, with the outcome being evidence-based practice.
  • Excellent IT, written and verbal communication skills.
  • Sound understanding of the legal and moral obligations related to client confidentiality and privacy.
  • Sound understanding of issues confronting children in care.
  • Working with Children Check.
  • National Police Clearance.
  • Form 395 Clearance from Department of Communities.
  • Drivers Licence.
  • First Aid Certificate.
  • Ability to work autonomously and as part of a multi-disciplinary team.

Desirable requirements

  • Extensive experience in working with or caring for children and young people who present with challenging or complex behaviours.
