About the Role

The Youth Case Manager will be responsible for the direct case management for young people under the Youth In Power program, and ensure contractual obligations are met. The Youth Caseworker will be responsible for the delivery of comprehensive intake assessments, engagement, transitions plans and continuous support for young clients. The role will have a focus on ensuring successful outcomes to increase the independence and self-reliance of young people at risk with a focus on CALD and ATSI individuals in the programs. A large part of this role includes outreach at identified local High Schools.

  • Complete holistic intake assessments, with a focus on client goals, strengths and rapport building
  • Coordinate case work, provide referrals to relevant stakeholders and follow up on client referral outcome
  • Enable referrals into education and capacity building activities to build independence and self-reliance
  • Ensure that all client case work data is current and up to date on client data systems; all case notes are completed and client files filed according to confidentiality and privacy principles.
  • Comply with all contract conditions of Youth in Power Program, including meeting specified client outcomes and completing detailed monthly reports in a timely manner. Provide reports, case studies as requested.
  • Develop and maintain collaborative partnerships with key stakeholders and internal programs to improve referral pathways for clients.
  • Promote and maintain systems for high level client service and feedback processes. Promote and Improve perceptions of ACSL and its programs within the community
  • Attend regular meetings with the youth, settlement and social service teams. Promote a positive and supportive organizational culture within the team.
  • Other duties as directed from time to time within your skills range and capacity, including at times work outside of office hours.

Selection Criteria

  • Tertiary qualifications in Human Services, Social worker or relevant Discipline Development/Development studies or relevant Discipline
  • Proven experience in working with young people from Culturally and Linguistically Diverse and or Aboriginal and backgrounds.
  • Experience in the delivering culturally responsive case work services
  • An understanding of skills and techniques to build clients capacity to become independent in Australia
  • Strong knowledge and understanding of the multicultural sector including relevant government policies and programs
  • Highly developed interpersonal and communication skills including the ability to deal constructively with complex and sometimes conflicting situations
  • Highly developed written and computer skills across a range of applications including client records management systems
