Our Client, is currently recruiting for an experienced Occupational Therapist with TACP experience to join their team based in Western Sydney

This role will involve:

• Undertake comprehensive occupational therapy assessment for older persons admitted to the LHD TACP program using agreed assessment and care planning pathways and measures, including carer and quality of life measures.
• Participate in the development and monitoring of individual comprehensive care plans for each client admitted to the TACP with clearly articulated and measurable functional goals.
• Provide occupational therapy specific interventions for clients within their home or in a clinical setting in accordance with professional standards and evidence based practice

The requirements for this role include:

• Available to commence between 20th Jan - 3rd Feb
• 5 days per week (open to 4 days if preferable)
• 8am-4.30pm
• pool cars available for working hours
• To cover secondment - 2 months with strong potential for extension
• Accommodation on site potentially available if required (relocating)
• Experience within the community and aged care desirable
• Relevant clearances and WWCC
• A tertiary qualification Occupational Therapy
• You must be registered with AHPRA

• A number of probity checks are required including holding a current working with children and undertaking a National police checks.
