The Organisation

Our client is an established Disability (NDIS) and Aged Care Service provider looking for a Disability professional to undertake reviews of all participants care plans as the Support Coordinator.

The Role

The Support Coordinator will be based in a dedicated team in the Inner West. You will be completing a wide range of tasks such as:

  • Coordinating supports for individuals engaging with this provider
  • Working with participants to build capacity, quality of life and independence
  • Ensure that all reports and service delivery in accordance of the NDIS framework
  • Build links for individuals to access mainstream, community and informal provider options
  • Negotiate services to be provided and their prices (including quotable services), develop service agreements and create service bookings
  • Work with participants to ensure supports and plans are of quality and meeting their needs
  • Supporting participants to identify and achieve their goals through their supports
  • Assist with plan reviews with participants
  • Have quality and value control that ensures all standards are met
  • Respect the privacy and dignity of all participants

The Successful Candidate

Essential:

  • High knowledge and understanding of the National Disability Insurance Scheme (NDIS)
  • Awareness of disability sector and people with disabilities
  • Experience in providing services to participants as a Support Coordinator
  • Experience in the assessment and monitoring processes for NDIS participants including other mainstream services
  • Completed risk assessments in the workplace safety context or willingness to learn
  • High level of verbal and written communication skills including skills for Microsoft Suite
  • The commitment of working with participants within a strengths-based, person-centred framework
  • Leadership skills and proven relationship management skills with internal and external stakeholders
  • Ability to work effectively, efficiently and accurately and prioritise workloads
  • Problem-solving skills and the ability to leverage networks
  • Australian Driver License

Desirable:

  • Relevant tertiary qualification
  • Experience working with participants alongside their family, support providers and communities to build capacity, quality of life and independence
  • 1 year + previous management or coordination experience in the disability, health/aged care, not for profit, or community services sector
