Our client, a well-known company with a reputation of providing top of the line service to their clients across NSW. They are one of the leading Not-For-Profit organisations in Aged Care. Due to organic growth in the business, they looking for an experienced Home Care Packages Support Advisor to work for their in-home Aged Care Service, empowering the lives of Older Persons and their families. This organisation is very well known and respected within the community as providing their clients with a high level of service and a family feel.

This role will involve:

Reporting to the Team Leader, you will be responsible for the assessments, referrals, service provision and ongoing case management of clients and their needs. Vital to this role is the promotion of the services to the community and initiating and maintaining ongoing contact with community organisations. You will have a strong awareness of WHS and a sound level of report writing and administrative skills.

The requirements for this role include:

• You will have had demonstrated experience as a Case Worker/Advisor in Home Care Packages or Disability Services and have a passion for building relationships and supporting aged people
• Experience or a qualification in community services, nursing or similar
• Outstanding written and verbal communication
• The ability to adapt to different clients and deal with complex client and family situations
• The ability to follow policies and procedures
• A current driver licence and own car
• As well as having a passion for helping people, you will be an advocate for the organisation and be able to represent the organisation and the great work they do.

Additional Information

• A number of probity checks are required including holding a current working with children and undertaking a National police checks.
