Our client, a well-known company with a reputation of providing top of the line service to their clients across NSW. They are one of the leading Not-For-Profit organisations in Aged Care. Due to organic growth in the business, they looking for an experienced Home Care Packages Support Advisor to work for their in-home Aged Care Service, empowering the lives of Older Persons and their families. This organisation is very well known and respected within the community as providing their clients with a high level of service and a family feel.
Reporting to the Team Leader, you will be responsible for the assessments, referrals, service provision and ongoing case management of clients and their needs. Vital to this role is the promotion of the services to the community and initiating and maintaining ongoing contact with community organisations. You will have a strong awareness of WHS and a sound level of report writing and administrative skills.
