Our Client, a leading Aged Care service provider, is currently recruiting for an experienced Care Coordinator to join their team based in the Eastern Suburbs.
This role will involve:
- Answering incoming enquiries
- Coordinate client services and case managers schedules on a daily basis
- Monitor and update client and case manager activities and follow up on any issues reported
- Coordinate and maintain client care plans on a monthly basis
- Assist Operations Manager and Care Services Manager with low level care management needs
- Help with recruiting new support workers
The requirements for this role include:
- 1-2 years' experience in a similar role
- Experience managing a fast-pace and rapidly changing roste
- Excellent data entry skills
- Personal and friendly approach to customer service
Additional Information
- A number of probity checks are required including holding a current working with children and undertaking a National police checks.
