Purpose/Objective:

To work with our clients to formalise a support plan and help them engage different supports to live more independently and be more included within their community.

Responsibilities and Duties

  • Working with a designated caseload to implement participant NDIS plans;
  • Ensuring that all operational and administrative requirements are completed including regular reporting and records maintenance;
  • Engaging participants in developing professional and trusting relationships;
  • Promoting the participants' right to choose and control by enabling them to have direct input in how their services are delivered within their identified plan;
  • Maintaining productive working relationships with stakeholders to ensure participant needs are met throughout the life of the plan.

Qualifications & experience

  • Strong knowledge of the NDIA, NDIS plans and funding categories;
  • Ability to work autonomously as part of an overall team;
  • Previous, sound experience working within the disability industry;
  • Police Check, Working with Children Check and/or an NDIS Worker Screening Check
  • Self-motivated with a flexible approach to work;
  • Completed NDIS Worker Orientation;
  • NSW Driver’s License with access to a comprehensively insured vehicle.
