Located in North Ryde, NSW the Dementia Support Specialist - Intake is responsible for supporting people living with dementia, their families and carers to enhance their quality of life through the provision of personalised information, support, referral, resourcing and innovative responsive service models. The position promotes wellbeing and builds self-efficacy through face to face, telephone, or online support and innovative service responses.

Reporting directly to the Team Leader, Client Services, you will also be responsible for:

  • Screening and assess clients to ensure that appropriate and timely services are received.
  • Providing a centralised intake system for Client Services.
  • Resourcing, supporting and promoting telephone and online based support services models including National Dementia Hotline.

To be successful in this role you will be a team player, show an absolute commitment to the delivery of high quality dementia services and support and meet the key selection criteria outlined below.

Key Selection Criteria:

  1. Experience in screening and assessment (beyond student placement).
  2. Demonstrated capacity to communicate and collaborate effectively within a team and with key service providers.
  3. Knowledge and understanding of the impacts of dementia for individuals, carers and their support networks.
  4. Excellent verbal, interpersonal and written communication skills, including the use of active listening and reflective questioning.
  5. Well-developed case note, record keeping and report writing skills.
  6. Excellent computer skills and knowledge of data entry.

Qualifications:

  • A formal qualification in a related field of either Social work, allied health, nursing and/or significant experience working with clients in the delivery of a community based service or within a health related field.
