The Dementia Occupational Therapist will assist and support people living at home with dementia, and their carers to understand the impact the environment can have on functioning and well-being; and will develop skills and strategies to overcome challenges and create a more dementia enabling environment. This role will support the development of a personalised intervention plan that will empower participants to make adaptations to their home that will increase independence, comfort, accessibility, and safety for the person living with dementia. The position will be working within the Nightingale Program which is a palliative care model, referring back to the nurses for clinical assessment as required. Reporting directly to the Team Leader Client Services you will also be responsible for:

  • Delivering high quality Occupational Therapy service to people diagnosed with dementia and/or their families and care partners
  • Undertaking screening and assessment of clients to ensure appropriate and timely service delivery
  • Using an enablement approach to empower clients to have an active role in directing their own care and support service
  • Developing, planning, and delivering individual and group information, education, and support about dementia friendly home environments
  • Facilitating referrals to other Dementia Australia services or external service providers
  • Ensure key outcomes and deliverables relating to funding agreements are consistently met, including timely data entry and reporting of outcomes and outputs
  • Supporting increased community capacity through building collaborative partnerships with other service providers and external stakeholders as appropriate
  • Maintaining professional knowledge and standards to ensure high quality service provision including aligning service delivery with Dementia Australia’s values and the overall direction of the program

Key Selection Criteria:

  1. Experience in person-centred care approaches when working with people living with dementia
  2. Demonstrated understanding of the impact of the environment on functioning and wellbeing for a person living with dementia
  3. Understanding of the range of technological and non-technological adaptions available to support improved independence and functioning in the living environment
  4. Ability to assess needs for adaptions to living environments suitable for a person living with dementia through remote engagement
  5. Excellent written and verbal communication skills and the ability to quickly build rapport and trust
  6. An engaging and dynamic presentation style

Qualifications:

  • BSc (Occupational Therapy) or equivalent
  • Registration or eligibility for registration with AHPRA
