At Uniting, our Case Managers are called Community Support Partners. This role is responsible for the case management of clients who have complex care needs, making a difference in the delivery of quality and collaborative Home and Community Care services. The Community Support Partner will utilise specialist strategies that are person-centred, to produce outcomes that enable clients to effectively participate in decision making related to their care, access services, and to keep them engaged in their care journey.
This is a full-time role, that can be based at our Orange or Bathurst office, supporting our Central West NSW area.
