The St Vincent de Paul Society is a leading provider of community support services and has been assisting people experiencing disadvantage in NSW for more than 130 years. The Continuing Coordinated Care Program (CCCP) provides specialist support and is part of the Health Team that is responsible for state-wide management of excellent health services. CCCP is a model of care for people in NSW who are experiencing AOD use issues. The Program's objective is to provide continuing care and intensive outreach support that assists people to access and maintain their engagement with treatment services while helping them to address other complex needs.
The Care Coordinator will deliver the services in accordance with the CCCP. Your knowledge and skills in the AOD sector will be key to being able to effectively provide tailored care support to individuals experiencing drug and alcohol issues.
