The Child and Family Program including Healthy Mothers Healthy Babies (HMHB) is comprised of numerous disciplines that focus on assessment and child and family focused care.

The program consists of Counsellors, Occupational Therapists, Speech Pathologists, Dieticians, Podiatrists, Psychologists, Physiotherapists, Exercise Physiologist, Community Development Workers and Family Support Workers. 

The team is committed to working with communities, families and individuals who have poor health outcomes and to providing high quality, multidisciplinary programs which embrace child and family centred practice, evidence-based practice, and the Social Model of Health.

The Navigator Clinician positions offer an exciting opportunity to work in a large and vibrant multidisciplinary paediatric allied health team in a community setting.

The role offers rewarding learning and development opportunities and regular clinical supervision.

There is scope for other activities such as quality improvement projects.

A Day in the Life:

We are seeking an experienced Occupational Therapist, Speech Pathologist and/or Social Worker to join our dynamic team into Navigator Clinician roles.

These roles would be suitable for Grade 2 and Grade 3 clinicians who wish to work in a position that is flexible, mobile and responsive to family needs.

The Navigator Clinicians will work across the City of Greater Dandenong, City of Casey and Cardinia Shire to provide assessment and service navigation to children and families, supporting children to gain access to NDIS Services (including Early Childhood Early Intervention Approach):

  • Opportunity to work in a large, multidisciplinary allied health team in community health
  • Conduct developmental screens, discipline-specific and/or transdisciplinary assessments
  • Collate evidence to support the NDIS referral process
  • Refer children to NDIS services (or equivalent) and support their access to long-term services
  • Provide short-term support to families to address developmental and social issues that are a barrier to engagement with NDIS
  • Attend meetings, clinical case review, in-service groups, supervision
  • Liaise and partner with key stakeholders
  • Advise and support the broader multidisciplinary team with referrals, service navigation and engagement
  • Quality improvement and service development

These positions are fixed-term until June 2024 due to temporary government funding.

The roles will be predominantly based at Springvale and/or Cranbourne, however some travel across sites will be required.

About You:

  • Minimum of three years experience (at least one in paediatrics)
  • Able to work independently to provide services to children, families and other stakeholders
  • Experience and training relevant to the role
  • Ability to work collaboratively with the multidisciplinary team
  • Attitude of lifelong learning
  • Values align with iCARE values (integrity, compassion, accountability, respect, excellence)

  • Relevant qualification (speech pathology, occupational therapy, social work)
  • AHPRA Registration (if required)
  • Eligible for membership with professional body
  • Please note a you will be required to complete a Police Check (or have valid Police Check)
  • Working with Children’s Check (required)
  • Current Drivers Licence without restrictions
  • Appropriate Australian working rights
