About the role

The Field Supervisor demonstrates organisational and leadership skills in the coordination of the Youth Advocate Program. The position is focused upon ensuring that the individual needs of the clients are met, whilst maintaining an efficient and safe working environment for Youth Advocates.

Key responsibilities

  • Undertake the role of Youth Advocate as per approved position description for a maximum of 30hrs per week for two clients and their families in the Youth Advocate Program.
  • Support and assist staff with design and implementation of individualised service plans, assess the needs of the client and maintain service records, communicate with appropriate individuals regarding the client and prepare written communications including the discharge reports.
  • Develop and monitor roster for the appropriate advocacy face-to-face hours for each Youth Advocate as specified by the service plan.
  • Assist Youth Advocate to develop a supportive relationship with each client's family in order to identify client needs and adapt to the changing circumstances of families.
  • Manage YAP documentation relating to service / case plans, progress notes and other documentation while ensuring the CIRTS database is up to date with all client information.
  • With the Manager, Specialised Programs, involve the appropriate community professional resources in the Youth Advocate Program.
  • Accurately complete a monthly report, timesheets and other required documentation and submit them to the Manager, Specialised Programs.
  • Facilitate and conduct staff development and training sessions as scheduled by the Manager, Specialised Programs.
  • Ensure that outcomes are captured, analysed and used to inform quality improvement of the program.
  • Adhere to all YAP, Inc. Personnel policies, the YAP, Inc. Code of Conduct and the Integrity Compliance Plan.
  • Develop knowledge of YAP, Inc. policies referring agencies' and third party payers' contracts and requirements and applicable governmental agencies' rules and regulation are adhered to and met in the performance of job duties
  • Integrate detailed knowledge of YAP program model and core principles into systems and processes that ensure achievement of program-fidelity.
  • Perform other related duties as assigned by the Manager, Specialised Programs.
  • Maintain a supportive and safe working environment.
  • Contribute to a positive team environment by demonstrating best practice support of clients, proactive interpersonal skills and peer support.
  • Support and promote the work of Life Without Barriers, maintaining a positive image of the organisation in accordance with level of position.
  • Comply with all Life Without Barriers policy, code of conduct, procedures and practices, external funding body requirements and legislation.
  • Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.

Skills and experience

Essential

  • Demonstrated ability to work with individuals, families and/or groups from a strength-based culturally appropriate perspective.
  • Sound team leadership experience including planning and delivering.
  • High level communication, influencing, liaison and negotiation skills.
  • Current Driver Licence.

Desirable

  • Diploma in Youth Work, Education, Communication, Management or related Social & Community Services qualification
  • Knowledge of the Human Services industry.
  • Experience in a similar role

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Children Check (where relevant to the role).
