• Hours: 30 hrs / week – Fixed Term until June 2024

We are looking for an enthusiastic and motivated Chronic Conditions Network Coordinator. This is a new position that will work to establish a Chronic Conditions Network for the ACT and region. The Chronic Conditions network will link local support services and groups, individuals, and health services, to work together to make connections, share information, identify advocacy needs and inform policy positions for people living with long term conditions. The role would suit someone with experience in community development and who can work both independently and as part of a collaborative team.

Our ideal candidate would have experience in:

• Community development and working in partnership with community organisations
• Preparing and evaluating health information for diverse audiences

• Contributing to health policy with specific reference to the needs of consumers
• An understanding of the social determinants of health and public health initiatives

This role contributes to the work of the HCCA health literacy and consumer participation teams.

The position is fixed term until 30 June 2024. Continuation of this position is dependent on funding.
