We leverage a predominantly volunteer workforce to deliver personalised, culturally appropriate services, meeting practical needs and strengthening community connection.

We recently moved into a brand new purpose-built facility, opening a new chapter in how we serve our community. As we grow, we're investing seriously in our people and systems, and that means investing in better client care.

About the Role

This is a newly created role. You'll be the dedicated point of continuity for the families we serve. The person who gets to know them, checks in on them, advocates for them, and walks alongside them throughout their journey with C Care.

You'll work alongside a Recipient Operations Coordinator who handles service logistics, so you can focus on what matters most: people.

Reporting to the Head of Recipient Services, you will:

  • Conduct comprehensive intake assessments with new clients and determine appropriate service packages
  • Deliver periodic check-ins with active clients to review circumstances and services
  • Respond to on-demand care needs as they arise
  • Liaise with and refer clients to external agencies for additional support
  • Manage client offboarding when they no longer need our services
  • Maintain accurate, timely client records and uphold confidentiality obligations
  • Assess service impact quarterly and develop case studies for learning and reporting
  • Help build our client triage framework alongside the Head of Recipient Services
  • Coach volunteers to support clients in a way that reflects C Care's values

About You

  • Degree in social work or a related field, with at least 2 years experience
  • Skilled at building rapport, assessing needs, and navigating the social services landscape
  • Strong documentation discipline and attention to detail
  • Familiar with mandatory reporting requirements under Victorian law
  • Comfortable working in a growing organisation where processes are still maturing
  • Genuinely curious about using AI and digital tools to improve your practice and extend your reach

Bonus points if you have familiarity with Jewish culture, speak Russian, Hebrew, or Yiddish.
