About you

The Health and Community Link Project is part of the Chatterbox Street Outreach service, which is an after-hours assertive outreach program that works with young people who are aged between 12 and 25, who are street frequenting, homeless, or at risk of homelessness and/or engaging in risk taking behaviour.

Purpose of Health and Community Link – Nurse (0.5 FTE)

The Health Link and Community Link – Community Health Nurse will work alongside the Outreach Social Worker, Chatterbox Outreach Worker, and Alcohol and Drug (AOD) Worker to alleviate issues associated with homelessness, and ensure young people (especially young women) can access sexual and reproductive health services, respond to issues associated with drug and alcohol misuse, family violence, and have access to employment and other day programs.

To be successful in this role you will be required to:

  • Relevant tertiary qualifications in nursing – Registered Nursing minimum qualifications.
  • Relevant practice experience – at least two years of registered nursing experience.
  • Registration with Australian Health and Practitioner Regulation Agency (AHPRA)
  • Hold a current Victorian driver’s licence with experience driving a manual vehicle, and must be willing to obtain a medium rigid driver’s license.
  • An understanding of the health needs of young people who have high and complex needs and the ability to establish and maintain relationships with individuals and families.
  • Experience and knowledge in offering health assessments and practical health care to complex clients, namely young people.
  • Advanced computer skills across the Microsoft Office Suite and highly developed verbal and written communication skills.
