Care Advisor


The Care Advisor is responsible for the assessment, coordination and delivery of quality care to our clients, ensuring that their physical, mental and social needs are being met.

Key responsibilities include:

  • Undertaking comprehensive assessment of clients and developing a care plans
  • Organising services, providing support and monitoring services to ensure quality
  • Ensuring the delivery of services meet all funding agreement, legislative and quality standards
  • Liaison with clients, referring agencies, family members, local health providers and community services to achieve the best outcome for the client
  • Supporting potential clients to navigate through the aged care funding system; on-board new HCP clients
  • Maintaining quality assurance and clinical risk procedures, and ensuring governance, quality of care and health and safety matters are addressed as a priority
