Outreach Case Manager

The Role:

VincentCare Victoria is seeking an appropriately-qualified person to take on the position of Outreach Case Manager to work with a range of clients in crisis including families, singles, couples and clients with complex health and housing needs.

Selection Criteria:

  • Appropriate qualifications in social work, community development or other related field;
  • Experience in case planning and support in a community based setting essential;
  • Knowledge of Government, community and private sector housing/support systems and associated issues;
  • Desire to work within a social justice framework and a commitment to effect positive change;
  • Highly developed written, verbal and interpersonal communication skills;
  • Advanced time management, organisational skills and high level of computer literacy;
  • Experience in using data reporting programs such as SHIP; VincentCare utilises its own client data system called Single Client Record.

All appointments within VincentCare Victoria are subject to satisfactory completion of a police check and character/performance reference checks.  Police checks will be undertaken for the selected candidate prior to any job offer being confirmed. Police checks are also undertaken on a periodic basis during the period of employment. 

All appointments are subject to disclosure of any relevant employment history of formal disciplinary action for improper or unprofessional conduct taken by current or previous employers or any other integrity body within or outside Australia.

The incumbent for this position must have and maintain a current Victorian Working with Children Check or equivalent.
