Queensland Positive People is seeking a person living with HIV (PLHIV) who is committed to support the PLHIV community through the Peer Case Manager role. This fixed-term full time role commences in October through to June 2021 with the possibility of extension.
Aiming to address social determinant barriers to HIV care and treatment through complex case management and ultimately improve PLHIV quality of life, the Peer Treatment Facilitator will also provide educational and emotional support for PLHIV newly diagnosed and reengaging in care. The role also requires skills and experience in providing complex case management support for those experiencing challenges and issues such as mental health, finances, housing, and employment.
