About the Position

This exciting position will attract candidates with a passion in providing support coordination to culturally diverse communities, participants and their families and to enable the individual to pursue their goals whilst coordinating, monitoring and reviewing their NDIS plans.

Key Functions of the Role

  • Service Tracking
    • Analyse and monitor delivery of services against NDIS plans
    • Liaise with NDIA regarding client funding
    • Administrative functions relating to service tracking
  • Service Design
    • Assist people with a disability to coordinate support services
    • Assist in the design and implementation of a range of systems and processes to compliment the functionality and success of the service
  • Service Delivery
    • Provide responsive and individualised services and support
    • Promote community inclusion of people with disabilities and their families
    • Develop culturally diverse partnerships
    • Provide Support Coordination services
    • Provide person centred support based on skills, competencies and resources to enable and promote the wellbeing and development of participants
    • Liaise with NDIA regarding revenue tracking and plan management

About You

  • Minimum 3 years experience in a similar position
  • Demonstrated understanding of NDIS, National Disability Service Standards and other relevant legislation
  • Tertiary qualification in a related discipline
  • Strong analytical and problem solving skills
  • Ability to conceptualise new and innovative initiatives
  • Excellent interpersonal, communication and administrative skills
  • Strong organisational skills with the ability to respond to demands in a timely manner
